WAIVER 2025
In consideration of registration with the Queens Falcons Youth Football Organization, Inc. D/B/A The Queens Falcons, I/we the parent or guardian of the name registrant, do hereby claim the child to be medically fit by way of physical examination and give my/our approval to their participation in any and all of the activities during the current football season. I/we assume all risk and hazards incidental to the conduct of the activities, its transition to and from the activities, and I/we do release, absolve, indemnify and hold harmless The Queens Falcons Youth Football Organization, Inc and any and all of its associated members, the organizers, the sponsors and the supervisors any and all of them. In case of injury to the child, I/we waive all claims against the organizers, the sponsors, and the supervisors appointed by them. I/we likewise release from responsibility any person transporting the child to and from the activities. All photos, videos, and other types of photographs are sole property of Queens Falcons Youth Football Organization, Inc. Queens Falcons Youth Football Organization, Inc. reserves the right to publish or utilize such photos, videos, etc. for promotional, advertising and other such uses as deemed necessary.
CODE OF CONDUCT 2025
- Talking to opponents is forbidden . No adult should be talking to a player from the opposing team
- All should be mindful of inappropriate language
- Harassing the officials only upsets them and gives the officials reasons to throw flags. At the end of the day officials are just people who want and should be respected
- Cheer for the Falcons without talking trash to the other team
- The other team’s behavior does not give Falcon spectators the right to respond in a similar fashion. The Falcons attempt to hold themselves to a higher standard
- Alcohol should not be consumed at a Falcon football game
- Parents “MUST” remain in an area assigned to view games – unless filming for the coach – holding the chains – assisting the coaching staff in some fashion
- At Mafera, double parking or parking on sidewalks or on the softball field is not permitted.. Police will be called and have your vehicle ticketed and/or towed. Doing this also puts the Queens Falcon’s permit for Mafera at risk
- No player should leave the practice/game without the coach’s knowledge. Either the Parent/Guardian or the child should notify the coach or staff prior to the departure. Failure to do so can lead to dismissal from the program!
- Most important: This is a game our players love, let’s give them the best support we can and make it a great memory. Let’s enjoy watching the game they love
Equipment distributed to registrants (helmets and shoulder pads) are property of The Queens Falcons Youth Football Organization, Inc.. Loss of equipment or failure to return equipment will require the registrants to reimburse the current cost of the equipment to The Queens Falcons Youth Football Organization, Inc.
Registrants are required to take reasonable care of their equipment. If it has been deemed that the equipment has been damaged by something other than normal wear and tear from football, the registrant will be required to cover the cost of reconditioning of damaged equipment.
*Upon request a certified copy of the above registrants birth certificate must be furnished.
